The Health Home Core Sets of quality measures for Medicaid are key indicators of the access to—and quality of—the health care that Medicaid health home enrollees receive. The Core Sets are important tools states can use to monitor and improve the quality of health care provided to Medicaid beneficiaries enrolled in a health home program.
Mathematica is supporting the Center for Medicaid and CHIP Services (CMCS) by convening the 2025 Health Home Core Sets Annual Review Workgroup to review and strengthen the Health Home Core Sets. In order to support states’ efforts to meet the upcoming 2024 mandatory reporting requirements and to provide sufficient time for states to prepare, the Centers for Medicare & Medicaid Services (CMS) released both the 2023 and 2024 Core Sets of 1945 Health Home quality measures for Medicaid based on the recommendations of the 2023 Health Home Core Set Annual Review. This year’s review will also assess the proposed 1945A Health Home Core Set measures for the new state plan option.

Orientation Meeting (Virtual)
This is the first meeting of the 2025 Health Home Core Set Annual Review Workgroup. This year’s review will focus on updates to the 2025 Health Home Core Sets. In order to support states’ efforts to meet the upcoming 2024 mandatory reporting requirements and to provide sufficient time for states to prepare, the Centers for Medicare & Medicaid Services (CMS) released both the 2023 and 2024 Core Sets of 1945 Health Home quality measures for Medicaid based on the recommendations of the 2023 Health Home Core Set Annual Review. This year’s review will also assess the proposed 1945A Health Home Core Set measures for the new state plan option.
This meeting will introduce the Workgroup members, review the outcomes of the 2023 Health Home Core Set Annual Review, provide an overview of the 2025 Core Set Review process, and prepare Workgroup members for the Call for Measures. This meeting is open to Workgroup members and the public. There will be opportunities for public comment during the meeting.
Meeting to Prepare for the 2025 Review (Virtual)
This meeting will summarize the measures suggested by Workgroup members for addition to or removal from the 2025 Health Home Core Sets, provide an overview of materials available to Workgroup members for reviewing the measures, and describe the format of the Meeting to Review Measures for the 2025 Health Home Core Sets. This meeting is open to Workgroup members and the public. There will be opportunities for public comment during the meeting.

Meeting to Review Measures for the 2025 Health Home Core Sets (Virtual)
In this multi-day meeting, the Workgroup will make recommendations for strengthening the Health Home Core Sets. Workgroup members will discuss and vote on the measures suggested for addition to or removal from the Core Sets and identify gap areas in the Core Sets. This meeting is open to Workgroup members and the public. There will be opportunities for public comment throughout the meeting.
