Introduction to D-SNPs and D-SNP Contracting and Using D-SNPs to Integrate Care for Dually Eligible Individuals
In this two-part Integrated Care Resource Center webinar series, staff from Mathematica and the Center for Health Care Strategies present on:
- The different categories of dual eligibility and the ways in which enrolling different categories of dually eligible individuals into D-SNPs can impact a state's ability to integrate Medicare and Medicaid benefits and streamline D-SNP enrollees' experiences of care;
- What D-SNPs are and how they differ from regular Medicare Advantage plans;
- The basics of D-SNP contracting with states, including required minimum contract elements; and
- The differences between Coordination Only (CO) D-SNPs, Highly Integrated Dual Eligible Special Needs Plans (HIDE SNPs), and Fully Integrated Dual Eligible Special Needs Plans (FIDE SNPs).
- State contracting and policy approaches to developing integrated care models in states with or without Medicaid managed care programs for dually eligible individuals;
- Useful contract provisions, beyond minimum elements, that states may use to drive improved care coordination for D-SNP enrollees and integration of Medicare and Medicaid benefits in D-SNPs; and
- Strategies for D-SNP quality monitoring and oversight
