The patient-centered medical home (PCMH) and the accountable care organization (ACO) are models of delivery system reform. Although there is theoretical alignment between PCMHs and ACOs, the discussion of physician payment within each model has remained distinct. This article compares payment for the two models, considers opportunities for integration, and discusses implications for policymakers and payers considering promoting primary care through ACOs.
