- To scale and spread efforts to address HRSNs, AHC awardees developed strategies focused on four drivers of lasting impact: data collection and use, strategic partnerships, organizational capacity, and health systems transformation and financing.
- Health care systems, payers, and community-based organizations seeking to initiate and support HRSN screening, referral, and navigation in clinical settings can consider the drivers of lasting impact and AHC awardees’ related strategies.
Drawing from AHC awardees sustainability plans, this resource highlights promising strategies and approaches to expanding HRSN screening, referral, and navigation efforts after AHC.
