This toolkit is a starting point for multisector community coalitions, partnerships, or organizations addressing social determinants of health to develop plans to use storytelling to communicate the impact of their work. It walks through the four elements of an impact story, including the why, or the vision and purpose; the what, or the key messages and metrics; the who, or the messengers; and the how, or the venues to share and sustain coalition efforts.
