This report summarizes the contributions of Collaboration for Community Health funded partners in removing obstacles to children’s health and well-being in Boston. Within eight equity-focused initiatives, the report examines funded partners’ progress towards improving children’s social determinants of health.
This annual report shares highlights from the first year of the new cycle of Birth to Five, Healthy Living, and Family Housing Stability and Economic Opportunity funded partners (2021-2024). It also shares the accomplishments of Mental Health, Youth Support Systems, and Community Trauma Response initiatives over a three-year period (2019-2022).
